距原始游戏的事件已经过去了20年。病毒获胜，人类正在慢慢灭绝。你将扮演艾登·科尔德威尔，一位流浪的朝圣者，负责运输货物、传递消息，并连接那些在被丧尸病毒摧毁的荒凉土地上生存下来的少数定居点。然而，你真正的目标是寻找你的小妹妹米娅，她是你小时候为了逃避沃尔茨博士的残忍实验而留下的。受到过去的困扰，当你得知米娅可能还活着并且在维尔多尔（地球上最后的城市）时，你决定面对这一切。你很快发现自己身处一个被冲突撕裂的定居点。你需要参与创造性且血腥的战斗，磨练技能，以击败成群的丧尸并结交盟友。在城市中漫游，穿越维尔多尔的建筑和屋顶，寻找偏远地区的战利品，同时小心夜晚。每当太阳落下，怪物就会控制街道。十五年前，人类因“陨落”这一灾难性事件而遭受重创，这一事件永远改变了世界。随着哈兰病毒在全球蔓延，人们迅速发现明天的希望已经丧失。到2036年，只剩下少数定居点，而人类正在慢慢灭绝，给无情的丧尸大军腾出了空间。
